Het jaar 2018 begon met het afscheid van Ruud Bosch, hoogleraar en voormalig afdelingshoofd. Er was een mooie bijeenkomst in Gasthuis Leeuwenbergh met voordrachten over epidemiologie, oncologie en functionele urologie, gevolgd door een afscheidsrede doorspekt met voetbalwijsheden. Helaas werd 2018 gekenmerkt door reductie van operatietijd door personeelstekorten. Met veel inspanning hebben we gezorgd dat de wachttijden tot operatie tot acceptabele lengte gereduceerd werden, onder andere door veel tweedelijnszorg naar Tergooi Ziekenhuizen te verwijzen. Een mooi voorbeeld van regionale samenwerking en passend in ons doel om vooral tertiaire zorg te verrichten. Nog meer hebben we ons gefocust op derdelijns incontinentie, urethrastricturen, derdelijns fertiliteitsproblematiek en complexe steenchirurgie.
Gewerkt is aan uitkomstenregistratie na urethrachirurgie (lokaal en nationaal) en na sfincterprotheses (internationaal). Een multidisciplinair fertiliteitspreekuur is tot stand gekomen en er werden structurele OK-slots gereserveerd voor de fertiliteitpatiënten. De percutane steenbehandeling werd nieuw leven ingeblazen.
Voor de medische vervolgopleiding werd het nieuwe curriculum geïmplementeerd, waarbij iedereen moest wennen aan de bekwaamverklaringen en de toevertrouwde professionele activiteiten. Met invoering van het nieuwe curriculum kiest iedere AIOS een differentiatie en is de opleidingsduur aangepast aan individuele vorderingen.
Op wetenschappelijk terrein is verder gewerkt aan de tissue engineering van de urethra. De FURU prijs voor het beste functioneel/reconstructieve abstract op een nationaal of internationaal congres werd gewonnen door Petra de Graaf met haar abstract Gel casting as an approach for tissue engineering of multilayered tubular structures: application for urethral reconstruction, gepresenteerd in Beijing, China. Verder werden in samenwerking met de TU Twente stapjes gezet in het project van de automatische analyse van flowcurves bij vrouwen. En het lange termijn onderzoek naar uitkomsten na hypospadiechirurgie naderde de eindfase.
De urologische staf wordt gevormd samen met oncologische urologen en kinderurologen. Met beide partijen vindt samenwerking plaats en onderlinge verwijzing. Deze samenwerking geldt ook voor het Centraal Militair Hospitaal. Andere partners waren revalidatiecentrum de Hoogstraat, Bergman Clinics voor vrouwenzorg en (ook voor de opleiding) Tergooi.
Per 1 mei heeft de afdeling Urologie weer een hoogleraar: Laetitia de Kort is benoemd op de leerstoel functionele, reconstructieve en adolescenten urologie.

Visie functionele urologie
Patiënten met klachten van disfunctie van de lage urinewegen worden in eerste instantie behandeld door de huisarts. Die besteedt aandacht aan levensstijlaspecten, nevenaandoeningen, medicatie en eventueel vochtintake. Ook kan de huisarts fysiotherapie aanbieden of medicatie voorschrijven voor de aandoening die op grond van leeftijd en/of geslacht en soms klinisch onderzoek het meest voor de hand ligt. Veel mensen zijn daarmee geholpen en tevreden.
Als de klachten met het ingezette beleid blijven bestaan of desondanks verergeren is de conclusie van de huisarts en waarschijnlijk ook van de patiënt: ‘Dus waren de diagnose en behandelkeuze mogelijk onjuist, onvolledig of niet precies genoeg.’ Verwijzing kan volgen voor nadere diagnostiek van de pathofysiologie die de klachten veroorzaakt. De urologische diagnostiek laat toe dat de dysfunctie objectief kan worden vastgesteld, urineweginfectie en neoplasmata goed kunnen worden uitgesloten en prostaatvergroting kan worden gekwantificeerd. Met uroflowmetrie, residumeting en urodynamisch onderzoek kunnen alle elementen van de disfunctie en onafhankelijk van de beleving van de patiënt worden gegradeerd. Natuurlijk worden levensstijl en kwaliteit van leven aspecten toch ook weer meegenomen in de 2e (en 3e)–lijn’s objectiverende diagnostiek en behandeling.
Urodynamisch onderzoek, meer of minder invasief, is voor de functionele urologie wat laboratoriumonderzoek en beeldvorming is voor de iedere klinisch specialist. De afwijking (disfunctie, in geval van klachten van de lage urinewegen) wordt zo nauwkeurig mogelijk gegradeerd en gestadieerd. Met name als –onomkeerbare- invasieve interventies worden overwogen en als relevante nevenaandoeningen bestaan is objectieve vaststelling van disfunctie noodzakelijk. Identiek aan de bijvoorbeeld oncologische stadiering is ook, dat met het verschuiven van therapeutische mogelijkheden én met het voortschrijden van kennis over afwijkingen de diagnostische testen, de stadiering en de gradering van de disfuncties mee -beweegt. Ook identiek is dat goede gradering van de disfunctie zinloze of overbodige interventies kan voorkomen en objectieve duidelijkheid kan geven aan patiënt en behandelaar.
Doel en missie van mijn klinisch werk en research binnen de functioneel diagnostische urologie zijn dus om het (urodynamische) onderzoek kwalitatief steeds te verbeteren en steeds beter aan te passen op de behandelmogelijkheden. Tegelijkertijd moet de diagnostiek steeds beter voldoen aan de wens van de patiënt tot zo min mogelijk invasieve maar wel objectieve en eenduidige vaststelling van de aandoening, die de symptomenverschijnselen en bezorgdheid veroorzaakt.
